내과계 중환자실 환자 체험에 대한 해석학적 현상학적 연구

Abstract

The medical intensive care unit is an essential place where vitals are managed. However, new unfamiliar instruments, limited space and family visit restriction are stress inducing factors. Therefore, in order to enhance the quality of MICU care, in-depth understanding of practical experiences of the admitted patients are required. The purpose of this study is to understand the in-depth meaning, using a hermeneutic phenomenological method, with respect to the lived experiences of the MICU admitted patients. The participants of this study were adult patients who had admitted to the MICU of Jeju National University Hospital. Eight male and three patients, 37-81 years old were included and their period of MICU admission ranged from 3 to 40 days. From 25th of April to 22nd of October, 2013, data were collected though in-depth interviews of the participants. And all the interviews were recorded and transcribed. The data were analyzed by phenomenological method of van Manen (1997). The study results were derived from four different findings of spatiality, temporality, corporeality and relationality. Lived space included 'place to meet the dead person', 'place to need to escape', 'painful sympathy of others' and 'place to presentiment death'. Lived time included 'time lost', 'time to reach agreement with life', 'time with uncertainty', and 'time with heartless closure and open'. The lived body included 'immovable', 'shame', 'struggle to communicate', and 'confirmation of living'. Lived others included 'regret', 'graciousness', 'people living without me' and 'returning to life, myself'. Participants experience were derived from four existence was escape out of the desert sand pit. 'Out of the desert sand pit' was experienced tremendous amount of pain, fear, and anxiety, but was revealed overcoming despair looking for a reason to live again, planning a new life. The results of this study lead us to understand the variety of pain and in what form the pain is presented. It shows the necessity of nursing care based on patients' lived experiences.
