Abstract
미국, 캐나다를 비롯해 영국, 덴마크 등 대부분의 유럽 국가에서 식도암과 위암의 발생경향이 변화하고 있다. 그중에서도 발생률이 매우 높았던 위암이 반세기 전부터 꾸준히 감소하고 있으며 특히 서구에서는 위암 발생률이 감소하는 반면 식도암의 발생률은 증가를 보이는 추세이다. 하지만 이를 위암 발생률의 변화로 살펴보면 분문부 이외의 위암은 감소하는 반면, 분문부 위암은 오히려 증가하고 있다. 이렇듯 인접부위이면서 암 발생경향이 비슷한 식도 원위부와 위 분문부의 일치성에 근거하여 최근 암 발생에 있어 원위부 식도암과 분문부 위암은 하나의 암이라는 가설 하에 원발부위 재분류에 대한 주장이 제기되고 있다. 현재 우리나라에서의 위암은 발생률과 암사망률에서 여전히 높은 순위를 차지하고 있어 한국인에서도 이러한 경향이 있는가를 살펴볼 필요가 있다. 따라서 기존의 식도암과 위암 분류 기준이 아니라 새로운 분류인 [Distal esophageal cancer + Gastric cardia cancer] 와 [Gastric non cardia cancer]로 재분류하고 분포를 비교하여 새로운 위암 분류를 적용하면 식도암과 위암 발생률이 년도 별로 어떻게 변화하는지 알아보고자 한다.
제주도지역암등록본부에 등록된 암등록자료로부터 초진일이 2000년 1월 1일부터 2003년 12월 31일까지 발생한 암 환자 중 식도암 환자 86명과 위암 환자 876명을 연구대상으로 선정하였다. 이를 세부부위별로 non distal esophagus (C15.0-C15.4), distal esophagus (C15.5), esophageal overlapping lesion & not otherwise specified (C15.8-C15.9), gastric cardia (C16.0), gastric non cardia (C16.1-C16.6), gastric overlapping lesion & not otherwise specified (C16.8-C16.9)로 나누어 조발생률(Crude incidence rate : CR)과 세계표준인구를 기준으로 한 연령 표준화 발생률(Age standardized incidence rate : ASR)을 산출하고 성별, 년도별로 비교하였다. 또한 원위부 식도암과 분문부 위암의 발생률을 식도암과 위암에서 주로 발생하는 squamous cell carcinoma군, adenocarcinoma군, 그 외는 other로 형태학별 분류하여 성별, 년도별 발생률을 비교하였다.
2000-2003년 식도암 진단을 받은 환자는 86명이었다. 그 중 남성에서 원위부 식도암 발생률은 2000년 10만 명당 0.41에서 2003년 10만 명당 0.34로 감소하였고 여성의 원위부 식도암은 발생건수가 없었다. 2000-2003년 위암 진단을 받은 환자는 876명으로, 그 중 남성에서 분문부 위암 발생률은 2000년 10만 명당 0.42에서 2003년 10만 명당 1.45로 증가하였고 여성에서 2000년 10만 명당 0.41에서 2003년 10만 명당 0.19로 감소하였다. [Distal esophageal cancer + Gastric cardia cancer]의 재분류 발생률은 분문부 위암의 발생경향과 비슷한 모습을 나타냈다.
서구와 제주지역의 원위부 식도암과 분문부 위암의 발생 경향이 일치하는 결과를 보이지는 않았으나, 부분적으로 유사한 경향을 발견할 수 있었다. 그러나 짧은 자료수집기간과 여러 제한점이 있어 원위부 식도암과 분문부 위암의 발생경향 및 부위 분류에 대한 더 많은 연구가 필요할 것으로 보인다.
The incidence rate of gastric cancer has declined in the world. Contrary to the general trend of steady decline in the incidence of gastric non-cardia cancer, many western countries have noticed an increasing occurrence in gastric cancer of cardia and esophageal cancer in recent decades. The cardia is the anatomical borderland between esophagus and stomach, while adenocarcinoma in the distal third of oesophagus and the gastric cardia have similar characteristics. So some studies insisted that distal esophagus and gastric cardia should be regarded and staged as one clinical entity.
Accordingly, the aim of this study is to evaluate trends in incidence rates for esophageal cancer and gastric cardia cancer according to subsite and histology over the period 2000-2003 in Jeju and to review the effects of reclassifing two regions by tumor location.
The subjects were obtained from the database of Jejudo Regional Population-based Cancer Registry. All patients were diagnosed newly as esophageal and gastric cancers from 1 January 2000 through 31 December 2003, while excluding malignant lymphomas, metastastic cancers, the cases of carcinoma in situ and the death certificate only (DCO). These esophageal cancers were anatomically divided into non distal esophagus (C15.0-C15.4), distal esophagus (C15.5), esophageal overlapping & not other specified (NOS) (C15.8-C15.9). Gastric cancers were anatomically divided into gastric cardia (C16.0), gastric non-cardia (C16.1-C16.6), gastric overlapping & not other specified (NOS) (C16.8-C16.9). Classification and analysis were based on the International Classification of Diseases for Oncology (ICD-O, third edition, WHO, 2000). Crude incidence rates (CR) of esophageal and gastric cancers in 4 year periods estimated by tumor location. The changes though the times were also evaluated independently and in aspect of sex. Age-standardized incidence rates (ASR), using the age standardized rates for world standard population, and ASR were computed by anatomic subsites, histology, and sex. SPSS 12.0 was used for statistical analysis.
Between the years 2000 and 2003 there were 86 cases of esophageal cancer in Jeju. In 2000, the ASR of distal esophageal cancer in men was 0.41/100,000; it declined to 0.34/100,000 in 2003. The ASR of distal esophageal cancer in women was 0 from 2000 to 2003. For gastric cancer, there were 876 cases between the years 2000 and 2003. In 2000, the ASR of gastric cardia cancer in men was 0.42/100,000; it increased to 1.45/100,000 in 2003. The ASR of gastric cardia cancer in women was 0.41/100,000 in 2000; it declined to 0.19/100,000 in 2003.
There was no evidence about increasing incidence of distal esophageal cancer in both men and women. But an increasing trend of esophageal cancer in both men and women and an increasing trend of cardia cancer in men and an deceasing trend of non cardia cancer in men were observed, as like as western countries.
The long-term and follow-up studies for this phenomenon would be warranted because this study has many limitations.
