폐경기 초로(初老) 여성의 결혼생활 만족도와 우울의 관계에서 노화와 죽음에 대한 태도의 매개효과

Abstract

본 연구는, 중년기 혹은 노년기와는 여성내분비학적 및 연대기적으로 구분되는 특성을 지니고 있는, 폐경기 初老 여성에서 육체적 건강과 심리적 안녕 및 사회 생활의 전반에 지대한 영향을 미치는 우울의 발현과 관련하여 중요한 관계적 심리요인(interpersonal factors)으로서 부부관계를 중심으로 하는 '결혼생활의 만족도'를 선택하고, 그와 함께 중요한 개인의 내적 심리요인(intrapersonal factors)으로서 생노병사라는 운명적 순환의 고리 속에서 거부할 수 없는 숙명적 과제들인 '노화에 대한 태도'와 '죽음에 대한 태도'를 선택하여 '결혼생활의 만족도'와 폐경기 초로 여성의 대표 증상이라고 할 수 있는 '우울'의 관계에서 '노화에 대한 태도'와 '죽음에 대한 태도'의 매개효과를 알아 보고자 하는 목적으로 진행되었으며, 연구의 필요성과 목적을 토대로 하여 다음과 같은 연구문제를 도출하였다.

연구문제 1. 폐경기 初老 여성의 결혼생활 만족도와 우울의 관계에서 노화에 대한 태도의 매개효과가 있을까? 연구문제 2. 폐경기 初老 여성의 결혼생활 만족도와 우울의 관계에서 죽음에 대한 태도의 매개효과가 있을까?

상기 연구문제에 대한 답을 얻기 위하여 제주대학교병원 노년여성클리닉을 방문한 만 50세부터 만 59세까지의 폐경 후 여성 130명을 최종 연구대상으로 하여, 설문지조사의 방식을 통하여 '우울 성향', '노화에 대한 태도', '죽음에 대한 태도', 그리고 '결혼생활 만족도'를 측정하였다.

본 연구에서 사용한 측정도구로는, 첫째, '우울 성향'을 알아 보기 위하여 Beck, Ward, Mock와 Erbaugh(1961) 등이 개발하고, 이영호와 송종용(1991)이 번안한 를 사용하였고, 둘째, '노화에 대한 태도'를 알아 보기 위하여, Laidlaw(2007)가 개발한 측정도구를 유정화(2010)가 번안·수정하여 자신의 연구 "노인 집단에서 일상생활능력의 제한이 우울에 미치는 영향"에서 사용한 를 사용하였다. 셋째, '죽음에 대한 태도'를 알아 보기 위하여, Thorson과 Powell(1988)이 처음 사용하고, 한국에서는 박춘석(1992)이 번역·사용하였으며, 그 이후 김명희(2007)가 자신의 연구"호스피스 자원봉사자의 죽음불안과 죽음에 대한 태도"에서 사용한 측정도구를 사용하였다. 마지막으로, '결혼생활 만족도'를 알아 보기 위하여, Roach(1975)가 부부의 결혼만족도를 측정하기 위하여 고안한 73개 문항의 측정도구를 Bowden(1977)이 타당화 연구를 통하여 재구성한 48개 문항의 를 유영주(1986)가 번역하여 자신의 연구"결혼만족도의 타당화 연구"에서 우리나라의 문화적 배경을 고려하여 분석·재구성한 를 사용하였다. 자료분석을 위한 통계처리는 제주대학교 정보통신원(Software & Library)의 SPSS 18 program을 이용하여 시행하였다.

본 연구를 통하여 얻은 결과의 핵심은 다음과 같다.

첫째, '노화에 대한 태도'는 '결혼생활 만족도'가 '우울'에 미치는 영향 에 부분적 매개효과를 가지고 있다. 둘째, '죽음에 대한 태도'는 결혼생활 만족도'가 '우울'에 미치는 영향 에 매개효과를 가지지 않는다.

본 연구의 주요한 연구결과와 그와 연계된 논의를 토대로 다음과 같은 결론을 도출할 수 있을 것이다. 즉, 폐경기 초로 여성들에게 있어서는 '죽음의 문제'보다 '노화의 문제'가 더욱 심각하고 중대한 영향력을 다양하게 지니고 있는 '우울'발현의 내적 심리요인이라고 판단되며, '우울'을 보이는 폐경기 初老 여성들의 임상 진료 및 심리상담에 있어서는 다양한 관계적 심리요인과 함께 중요한 내적 심리요인으로서 '노화에 대한 태도'를 비중있게 고려할 필요성이 크다.

The aim of present study was twofold. Firstly, to investigate whether attitudes on senility mediate the effect of marital satisfaction on depression in presenile climacteric women. Secondly, to investigate whether attitudes on death mediate the effect of marital satisfaction on depression in presenile climacteric women.

Study problem 1 : Is there some mediating effect of attitudes on senility between marital satisfaction and depression in presenile climacteric women? Study problem 2 : Is there some mediating effect of attitudes on death between marital satisfaction and depression in presenile climacteric women?

In order to resolve the dual study problems, 130 active participants who were in her age of 50s, carring on present married status and receiving postmenopausal hormone replacement therapy at Senile Women's Clinic in Jeju National National University Hospital were recruited.

Firstly, measuring the propensity to depression, Beck's Depression Inventory(BDI)was adopted and secondly, measuring attitudes on senility, the attitudes to ageing questionnaire(AAQ) which was developed by Laidlaw(2007) and used by Yoo(2010) was adopted. Thirdly, measuring attitudes on death, the attitudes to death questionnaire(ADQ) which was developed by Thorson & Powell(1988) and used by Kim(2007) was adopted. Lastly, measuring marital satisfaction, korean marital satisfaction scale assessed by Lee(1986) from western version which was originally by Roach(1975) and later modified by Bowden(1977) was adopted.

Collected dada were analyzed with SPSS 18 statistics program and the kernel of the results were as follows.

Firstly, the effect of marital satisfaction on depression in presenile climacteric women was partially mediated by attitudes on senility. Secondly, the effect of marital satisfaction on depression in presenile climacteric women was not mediated by attitudes on death.

Based upon the results and discussions associated with the research subjects, following conclusion could be drawn. That is, in presenile climacteric women, apprehensions concerning senility would be more serious and influential factor than those concerning death in the advent of depression. So, therefore, practicing medical management or psycological counselling for presenile climacteric women with the problem of depression, their intrapersonal attitudes on senility should be taken into account as well as various other interpersonal psychological factors.
