A Comparison of Laryngoplasty Using Fiberwire and Securos Equine Suture in Thoroughbred Race Horses
Abstract
Laryngeal hemiplegia is a respiratory disease caused by paralysis of aryteonid cartilage. As a result, the function of adduction and abduction of the arytenoid cartilage is lost. The most preferred surgical method is a combination of laryngoplasty and ventriculocordectomy. Laryngoplasty, called tie-back, is the surgery to fix the arytenoid cartilage by abduction, and ventriculocordectomy is the surgery to remove the ventricle and vocal cord. Laryngoplasty is a method that binds the muscular process of the arytenoid cartilage to the cricoid cartilage. Among the threads used, the representative is fiberwire, which is a non-absorbable suture. However, it was reported that there was a loss of tension at the suture site after surgery, and the need for a replacement thread was required. Recently, the surgery by using a canine cranial cruciate ligament repair system (CCCLRS) instead of using fiberwire, has been developed. Laryngoplasty using CCCLRS has been shown to maintain abduction of arytenoid cartilage without recurrence of laryngeal hemiplegia. Securos equine suture was commercialized as a product for using CCCLRS in horse laryngoplasty. Therefore, we tried to compare the recurrence, return to racing, changes in exercise capacity, and complications. In 12 horses diagnosed with laryngeal hemiplegia, the laryngoplasty and ventriculocordectomy were performed. The 6 of 12 hoses were operated using fiberwire, the other 6 horses were operated using securos equine suture. After laryngoplasty, the recurrence and grade of laryngeal hemiplegia was evaluated with endoscopy. In the SE group, 1 horse is confirmed recurrent laryngeal hemiplegia after surgery. On the other hand, in the FW group, 2 horses 7 days after surgery, 5 horses 14 days later, and 6 horses 3 months after surgery all had recurrent laryngeal hemiplegia. According to the results, it is better to use securos equine suture than to use fiberwire as a method to prevent recurrence in laryngoplasty. This is because the effect of the tension of the thread, and the method of maintaining the tension by compressing the securos equine suture using a clamp, whereas in fiberwire, a surgeon's knot is used. In this study, the abduction of arytenoid cartilage was better maintained when securos equine suture than fiberwire was used in equine laryngoplasty. In the future, more clinical studies are needed.
