노인의 인지기능 저하의 관련 요인과 인지기능 저하가 노인의 삶의 만족도에 미치는 영향 : 2014년, 2017년, 2020년 노인실태조사 자료 활용

Abstract

목적: 본 연구는 우리나라 노인들의 특성에 따른 인지기능 저하 추이와 관련 요인을 파악하고 인지기능 저하가 노인의 삶의 만족도에 미치는 영향을 파악하여 인지기능 저하가 있는 노인들의 관리와 예방을 위한 기초자료를 제공하기 위함이다.

방법: 본 연구는 2014년, 2017년, 2020년 노인실태조사 원시자료를 활용하였다. 인지기능 저하 추이를 알아보기 위해 조사에 참여한 30,847명 중 대리응답, 치매 의사 진단, 인지기능 측정 불가, 우울 측정 불가를 제외한 최종 연구대상자는 29,686명이다. 분석 방법은 IBM SPSS statistics ver 24.0 프로그램을 이용하여 분석하였고 통계적 유의수준은 .05로 설정하여 검정하였다.

결과: 첫째, 인구사회학적 특성에서 연령은 85세 이상, 교육수준은 중학교 졸업, 연 가구소득은 제1오분위, 제3오분위에서 인지기능 저하가 증가하였다. 건강관련 특성에서는 평소 운동을 안 하는 노인과 우울이 있는 노인, 도구적 일상생활수행능력에 도움이 필요한 노인이 인지기능 저하가 증가하였다. 사회·경제활동 특성에서 동호회와 친목단체에 참여하지 않는 노인, 노인복지관을 이용하지 않는 노인이 인지기능 저하가 증가하였다. 경제활동 여부는 현재 일을 하지 않는 노인이 인지기능 저하가 증가하였다. 둘째, 2014년, 2017년, 2020년 노인의 특성에 따른 인지기능 저하 추이는 2014년에 비해 2017년은 인지기능 저하가 감소하였다. 하지만 2014년에 비해 2020년은 인지기능이 증가하였다. 셋째, 노인의 인지기능 저하가 노인의 건강상태 만족도, 경제상태 만족도, 사회·여가·문화활동 만족도 모두 유의한 영향을 미쳤다.

결론: 본 연구는 횡단적 조사 연구로 인지기능 저하의 관련요인 간에 선후관계를 추론하기에 제한이 되어 인지기능 저하의 원인을 명확하게 파악하기 위해서는 종단적 연구가 필요할 것이다.| Purpose: The purpose of this study is to provide basic data for the management and prevention of the elderly with cognitive impairment by identifying the trend of cognitive impairment per the characteristics of the Korean elderly people, the pertinent factors, and the effect of cognitive impairment on elderly life satisfaction.

Methods: This study used raw data from 2014, 2017, and 2020 surveys of the living conditions and welfare needs of Korean older persons. Of the 30,847 participants of the survey, which is to find out the cognitive impairment trend, the final study subjects were 29,686; surrogacy, responses from those who are diagnosed with dementia, and unmeasurable responses due to cognitive impairment or depression were excluded. For the analyses, The IBM SPSS statistics ver 24.0 program was used; for testing, the statistical significance level was set at .05.

Results: First, cognitive decline increased in the following demographic characteristics: for age, over 85 years old; for education level, middle school graduation; for annual household income, the first and third quintiles. Among the health-related characteristics, people with increased cognitive impairment were the elderly who did not exercise, the elderly with depression, and the elderly who needed help with instrumental activities of daily living. The socioeconomic activity characteristics of people with increased cognitive impairment are the elderly who do not participate in societies or groups, and the elderly who do not go to senior welfare centers. Regarding economic activity, cognitive impairment increased in the elderly between jobs. Second, the trend of cognitive impairment per the characteristics of the elderly in 2014, 2017, and 2020 is as follows. Cognitive impairment decreased in 2017 compared to 2014; conversely, cognitive function increased in 2020 compared to 2014. Third, the cognitive impairment of the elderly had a significant effect on all of the following variables: health status satisfaction, economic status satisfaction, and social, leisure, and cultural activity satisfaction of the elderly.

Conclusion: This study is a cross-sectional investigation study; there are limitations in inferring a causal relationship between the factors pertinent to cognitive impairment. Longitudinal studies will be needed in the future to identify the causes of cognitive impairment with more clarity.
